Your Patient Navigator

You will have your own CancerHome Patient Navigator who will coordinate with your caregivers to allow better collaboration from diagnosis through survivorship. This key point person will provide direction, support, and serve as a go-to resource for information on wellness services, financial questions, transportation, and support groups. In addition, this person is able to address any other individual needs. If you have clinical questions or concerns, your Patient Navigator will work with the cancer care team to provide the answers you need.
